Case Manager

HOURS: Full Time, 40-hour work week: Monday – Friday. Some evening and weekend work may be required- flexible.
 

SUMMARY: Provide individualized and comprehensive case management and advocacy services to victims of domestic violence; Develops a therapeutic relationship, assesses strengths and needs, creates goals, implements and monitors case management plan of Emergency Shelter clients
 

POSITION REQUIREMENTS: Minimum requirement of a Bachelor’s degree in social work, counseling, or related discipline from an accredited program. Minimum of one (1) year of case management related experience.
 

Experience in a residential setting preferred. 

Must have a knowledge of domestic violence as well as appropriate trauma-informed interventions.
Must be able to work effectively with diverse groups of clients, staff and individuals in the community.
Ability to work with minimal supervision, as well as in a team environment.
Ability to handle multiple projects simultaneously; work both independently and in a team environment and effectively lead, inspire, mentor, and develop a goal-oriented culture.
Ability to express complex information and ideas both verbally and in writing.
Ability to work within agency mission, philosophy and policies/procedures.
Must have a reliable vehicle and meet agency driving policy requirements, including a valid driver license and proof of required automobile insurance. Travel may be required.
General physical requirements include but are not limited to: bending, stooping, climbing stairs, prolonged sitting of more than 4 hours.

ESSENTIAL DUTIES INCLUDE BUT ARE NOT LIMITED TO:

CASE MANAGEMENT
Ensure appropriate and eligible victims of domestic violence are admitted to shelter. 

Provide case management and advocacy services for residential clients. 

Develops and conducts one psychoeducational group for clients that teaches life skills and/or social skills on a weekly basis. 

Conduct and/or ensure that intake assessments are completed within 24 hours of admission and Continued Intake is completed within 72 hours of admission. 

Provide direct services and referrals to potential and current program participants. 

Complete service plan with each shelter resident and monitor progress through weekly case management meetings. 

Screens telephone calls, provides crisis intervention, and makes appropriate referrals. 

Assist shelter clients with daily activities. 

Maintains a safe, secure, and non-violent environment for all clients receiving services. 

Make appropriate referrals to clients upon discharge from shelter. 

Provide client transportation as needed in agency vehicles. 

Accompany clients to court/legal proceedings as needed. 

Serve as liaison with community agencies and referral sources to better serve clients. 

Provide presentations and professional trainings, and conduct public relations activities as requested. 

Provide on-call support to shelter per established schedule.


AGENCY COMPLIANCE
Maintains accurate and thorough documentation in a timely manner reflecting all case management functions and activities in client database. 

Adhere to the agency’s confidentiality policies, maintaining a high-level standard of confidentiality. 

Report any known or suspected child abuse to immediate supervisor. 

Report any suspicious activity or persons. 

Attend and participate in agency staff meetings and trainings. 

Report to work on time as scheduled. 

Maintain a valid driver license and proof of required automobile insurance.

Perform all other duties and/or participate in special projects as assigned by Case Management Team Lead, Director of Program Development or Executive Director.


OTHER
In case of emergencies and extreme circumstances, may be required to assist with 24-hour shelter coverage. 

Work as a member of the multidisciplinary team and in conjunction with other program components and staff to ensure effective communication of agency and client concerns, recommendations, and needs.
